Residential Care

56026. (b)- Level 2 and 3- written semi-annual report on progress towards achievement of each IPP objective for which the home is responsible.

(d)- Level 4- written quarterly reports of progress towards the achievement of each IPP objective for which the home is responsible-administrator shall submit the quarterly report to the service coordinator within 30 days of the end of the quarter.

Supported Living

There are no regulations stating the frequency of written reports. SLS vendors usually reference frequency is their program design. Typically the vendor writes up a quarterly report to coincide with the required quarterly RCEB consumer contact.

ILS, Adult Day Program, Children’s After School Program and Infant Program,

56720- (a) (all non-residential vendors excepting respite providers) shall be responsible for establishing, maintaining, and modifying, as necessary, documentation regarding the manner in which it will assist each consumer served in achieving his/her IPP/IFSP objective (s) for which the vendor is responsible (this usually takes the form of an ISP).

(c) prepare semi-annual written reports

(1) review progress in relation to each IPP/IFSP objective

(2) submit reports to regional center with

· evaluation of progress

· identification of barriers with plans to overcome in future

· and whether the vendor’s program can continue to meet the needs of the consumer as specified in the IPP/IFSP.
